Roanoke County Enrollment Forms
REGISTRATION FOR THE 2020-2021 ACADEMIC YEAR WILL RESUME AFTER JULY 6TH, 2020.
You may print the Registration/Enrollment Forms found at the bottom of this page. If you are unable to print these forms, please call the CSMS Front Office (540-772-7560) AFTER July 6th and make an appointment to pick up an Enrollment packet. When you call, please provide the following information:
- Student Full Name
- Grade Level for 2020-2021 academic year
- Student Address
- Parent Name and Contact Number
Once you return your completed enrollment forms (by email to: firstname.lastname@example.org, fax (540) 772-2195, or dropping them off at the school), your information will be provided to a Counselor who will contact you regarding completion of the registration process.
In addition to submitting the forms that you have printed from this website (or picked up from the office), you will be required to provide:
- Proof of Residency (lease, deed, mortgage statement or Roanoke County Real Estate Tax Assessment)
- a copy of your student’s Birth Certificate
- a copy of your student’s current Immunization Records
