Senior Parchment (Transcripts) Information Form 

*Parchment is our electronic delivery program in which our counseling department uses to send transcripts to colleges for applications. Parchment's Terms & Conditions

 Ordering in Parchment - Initial - Directions   

Senior Release of Information Form for College Transcripts - This form is needed to be signed and returned to the school counseling office prior to transcripts being sent electronically via Parchment. Parent signature is required if under the age of 18. If students are over 18 years of age they can sign and return.
