Kindergarten Registration Forms 2023-2024
Welcome to Back Creek Elementary!
We are excited to have your student with us. Along with the forms attached, below are the items you will need to provide at registration time for your student:
-Original Birth Certificate
-Proof of Residency (Mortgage statement or lease signed by you and the landlord
-Immunizations and Physical within 1 year to the start of school (If you have an appointment for this please let us know and you can bring it to us later)
